Grant Guidelines for Therapy Providers

• Therapy provider must present documentation confirming they are a licensed Speech and Language Pathologist and ASHA certified.

• Therapy provider and the Site Supervisor (if applicable) must sign the contract.

• Per the contract guidelines, therapy providers will submit progress notes and invoices every three months.

• All invoices must be on professional billing letterhead.
